Canada Pension Plan Disability Application

There are a number of forms you will need to apply for CPP Disability, which you can find on the linked federal government site. You can apply online using a My Service Canada Account or using paper copies. The application guide will help you decide what information you need to include. Call the toll-free number if you have any questions about the application or whether you qualify for benefits. Benefits are available to people who are unable to work because of disability and who have paid enough in CPP premiums to qualify. Be aware that CPd does not include any coverage for medical equipment and supplies or medication.
